To access this system you first need to obtain Log In details from one of our Reception Team at either Ambrose or Tollgate, you will be asked for PROOF OF ID before we can give you the login details. Once you have your log in details come back to this website and click on the following icon to book appointments online, request repeat prescriptions, amend address details or to view your summary care record:
Please note this service will be for ROUTINE APPOINTMENTS ONLY. Any speciality appointments (such as coil fittings, travel vaccines, nexplanon fitting, ear syringing etc) will have to be booked by speaking to one of our Reception team. This service will NOT allow booking of DOUBLE APPOINTMENTS - if you need a double appointment please speak to a member of our reception team.

The practice premises at both Ambrose Avenue and Tollgate Health Centre have suitable access for disabled patients. There is one designated parking space for disabled patients at Ambrose Avenue and four at Tollgate Health Centre.
